Clinical Services - Cardiac Rehabilitation
Phase II: This phase of Cardiac rehab combines exercise training and wellness education for those who have had any of the following:
- Heart attack
- Stable angina
- Procedures performed on their hearts, such as stents, bypass surgeries or valve replacements.
It is supervised by RNs who stay in communication with the referring and family physicians. Patients wear a telemetry unit during exercise and are monitored for:
- Heart function
- Weight
- Oxygen saturation
- Blood pressure
- Blood sugar
Phase III: This is designed as a supervised exercise and education program that includes the following:
- Patients who have graduated from Phase II Cardiac Rehab.
- Patients who have risk factors for coronary artery disease and would benefit from a supervised exercise program. Risk factors include: high blood pressure, diabetes, stress, obesity, smoking, sleep apnea, family history of heart disease and chronic irregular heart rhythms.
- Those with chronic illness, such as: Chronic Obstructive Pulmonary Disease (COPD), Emphysema, Congestive Heart Failure (CHF) and Peripheral Vascular Disease (PAD/PVD).
Pulmonary Rehabilitation - Outpatient pulmonary rehabilitation is an important addition to the treatment of COPD and other chronic lung diseases such as asthma and interstitial pulmonary fibrosis. The Pulmonary Rehab Program at CMH uses a multi-disciplinary approach that emphasizes personal goals, which ultimately will allow patients in the program to manage their daily activities. The program attains these results through a personalized program of exercise, education and symptom management.
The pulmonary rehabilitation program is designed to help pulmonary patients:
· Understand causes, effects, risk factors and management of pulmonary disease
· Improve their ability to exercise, work and perform daily activities
· Prevent infection and promptly recognize signs when they occur
· Decrease hospitalizations through improved health
The program accomplishes the goals by:
· Evaluation and treatment planning
· Education of the disease process
· Breathing retraining
· Exercise reconditioning
· Home respiratory regime
· Medication information
· Energy conservation techniques
· Dietary management
· Stress management
· Oxygen therapy
Wellness Classes also include:
- Cooking demonstrations
- Wellness presentations
- Community educational opportunities, such as: Better Breathers Support Group, Diabetes Education Classes, and Cardiac Wellness education classes.
The Wellness Center also offers a supervised exercise program for those individuals with chronic health conditions. The emphasis is on building strength, flexibility, and cardiovascular endurance in a safe, supportive environment. An exercise specialist and RN will manage your exercise sessions and collaborate with your health care provider in order to help you reduce your risk of future heart problems.
Who: Anyone who has heart disease or risk factors for heart disease including obesity, high blood pressure, diabetes and high cholesterol.
When: Monday - Friday, 7:30 a.m. to 5 p.m.
