Rehabilitation Services

At Cheboygan Memorial Hospital (CMH) the long-term well-being of every patient is our top priority. We provide outpatient and inpatient speech, occupational and physical therapy services for all ages. Our highly skilled, licensed therapists are committed to creating a customized treatment program for each patient that will address the immediate rehabilitation needs while helping the patient on the path to previous/new levels of function. We listen to patient concerns and provide a comfortable, caring environment for setting goals, restoring health and movement, and establishing a life-long plan for wellness. Services provided include:

Physical Therapy (PT): Physical Therapy assists patients gain independence by restoring function and improving mobility. A physical therapist will evaluate a patient's strength, range of motion, endurance, balance, and ability to perform functional mobility, such as getting in and out of bed, transferring, walking and stair climbing. The physical therapist or physical therapist assistant will provide additional treatments. Recommendations are made for durable medical equipment that the patient may need for safety at home (cane, crutches, walker, wheelchair, etc.). The physical therapists can follow the patient across the continuum of care.

Occupational Therapy (OT): Occupational Therapy enables people of all ages to live life to its fullest by helping them promote health, prevent - or live better - with injury, illness, or disability. When providing occupational therapy services, clinicians address life activities of the patient including Basic Activities of Daily Living (BADL's), such as showering, dressing or feeding and Instrumental Activities of Daily Living (IADL's), such as home management, meal preparation, and community mobility. OTs may assess how a person performs their ADL's by looking at such areas as, strength, balance, vision, and cognition. They may suggest adaptive equipment or home modifications to improve performance and safety.

Speech Language Pathology (SLP): Speech-language pathologists assess, diagnose, treat, and help improve speech, language, cognitive-communication, voice, swallowing, fluency and other related disorders. Speech-language pathologists are involved with patients who have difficulty communicating due to their inability to comprehend others or produce speech. Patients with attention, memory, and problem solving disorders may receive speech therapy. Those patients who are having difficulty with swallowing also receive evaluation and treatment to ensure adequate nutrition through a safe diet plan.

Practice Settings: Services provided at CMH include: inpatient hospital care; sub-acute, long term care and outpatient.

  • Inpatient. When someone is a patient at a hospital they may need therapy to assist their return to their previous functional level. Treatment is provided within the hospital setting. It is scheduled following an order being written from the referring physician. The inpatient rehabilitation team provides care to patients of all ages and to a variety of diagnoses. Recommendations may be made for follow up care and therapy needs.
  • Sub-Acute Setting. Patients may be recommended to follow up with additional rehabilitation after their hospital stay. Within the sub-acute setting at the Larson Hall extended care unit, patients are able to continue to develop their prior functional level in order to safely discharge home. Patients are able to receive care from all the applicable therapies in order to provide the plan of care to restore their functional level. Family training is an important part of rehabilitation in a sub-acute setting in order for families to assist their loved one when they return home. Further recommendations may be made for additional rehabilitation with home health care or outpatient services when applicable.
  • Outpatient. Patients may be referred for additional rehabilitation within an outpatient setting. When the individual is living at home and able to have transportation to the clinic they may receive care from the therapy team. Within this setting, the therapists are able to optimize the patient's functional level of independence and quality of life. Patients who are treated within outpatient basis span from pediatric to adult.
